I’ve tried several times to resolve the issue and have yet to find the cause. I’m going to keep working on it and will update the post when resolved. No updates were made to the site when this broke, so that makes it even harder.<\/p>\n\n\n\n

UPDATE: Reverting to a previous version of the plug-in has addressed the problem while we work with the vendor.<\/p>\n\n\n\n
FINAL UPDATE: The vendor has updated their application to work with our type of configuration (which is not very common) and that resolved the issue.<\/p>\n\n\n\n
